Unlike a geriatric care manager who focuses on managing and overseeing the current plan of care. Maptic Nursing is advisory: you get the expert clinical read and advice on the next best steps to take. The work is made real through the Maptic Care Map, a clear, always current brief that connects your loved one’s medical team, their daily care team, and your family’s goals for the loved one.
Led by a doctorally prepared gerontological nurse practitioner
Tammi Harris, DNP, GNP-BC, is a board certified gerontological nurse practitioner with 23 years of hands on experience across the settings of assisted livings, board and cares, homebound, skilled nursing care, and hospitals. The depth of clinical training is the difference between someone coordinating tasks and someone who can actually read what is happening and where it’s heading.
